THE local government of Mabalacat City in Pampanga will start to issue official COVID-19 vaccination cards for its residents.
Mabalacat City Mayor Crisostomo Garbo, together with Vice Mayor Gerald Aquino and COVID-19 Response Integrated System (CRIS) chief implementer Dr. Jose Antonio Rivera, led the walkthrough of the claiming process at the designated Mabalacat City Official Vaccination Card (MCOVC) releasing station at the 2nd floor bridgeway between the Cyberzone and Wellness Center of the SM City Clark.
Rivera said individuals who were vaccinated with first and second doses in any of the designated vaccination sites in the city or within SM City Clark cinemas are eligible to claim their MCOVC.
The MCOVC contains a Quick Response (QR) Code which, when scanned, may present authenticated vaccination records from the database of Mabalacat City Management Information System (MISO).
Residents applying for the official vaccination card must secure the Mabalacat City Temporary Vaccination Card (MCTVC) and one government-issued or valid identification cards such as driver’s license, passport, GSIS/SSS card, UMID card, PRC ID, Philippine ID (PhilSys), barangay or company ID.
Individuals who lost their MCTVC must present a duly notarized affidavit of loss.
An authorized representative may claim a maximum of three MCOVCs on the owners’ behalf by providing an authorization letter, the MCTVC of the owner, and valid IDs from the owner and the representative.
For companies, an advanced bulk printing of the MCOVC is accepted, subject to the submission of an authorization letter from the company of the requestor, list of names of their employees claiming for MCOVC with contact numbers, and a photocopy of ID of the requestor.
Meanwhile, all MCTVC must be surrendered onsite after processing and upon receiving the official vaccination card, which may only last from three to five minutes.
The Mabalacat City OVC Releasing Station is open from Wednesday to Sunday from 10 a.m. to 3 p.m.
Joining Mayor Garbo and Vice Mayor Aquino in the inaugural launch were councilors Cocoy Tiglao and Carlo Dizon, city information officer Jay Pelayo IV, and employees and teachers from the Department of Education.
